Application 2024-25

To begin your online application for 2024-25, you will be asked to open a new account or login to an existing account.

PRINT the required forms below and submit them to your child’s current school. Once completed, your child's school should mail these forms to St. Agnes Academy.

Release of Records (applicants to 1^st – 8^th grades)

Teacher Recommendation (applicants to PK-4 and Kindergarten)

Teacher Recommendation (applicants to 1^st – 8^th grades)

Principal Recommendation (applicants to PK-4 – 8^th grades)

*These forms are NOT required for applicants to PK-3

The application deadline is January 31, 2024. If submitted after this date, applications will be considered as space permits.
